For further analysis, we had performed x-ray to the component, and no abnormalities was found during the x-ray. Solder looks good with GND pad 70% covered by solder. No solder shorts or insufficient solder on each pin. Notice that only rev F will causing the programming failed. All unit with rev H able to complete the programming.

With this we suspect that U6 rev F is the root cause to the failure. By replacing this component, unit was able to Pass board level test & module level test.
